Carefully read and understand the following before signing your name. Animal Allies uses qualified staffing and approved materials for all procedures performed. It is important for you to understand that the risk of injury or death, although extremely low, is always present just as it is for humans who undergo surgery. I, acting as owner of the pet named above, hereby request and authorize Animal Allies, through whomever veterinarians and assistants they may designate, to perform an operation for the sexual sterilization of the pet. I understand that some factors significantly increase surgical risk, including but not limited to pregnancy, animal 5 years and older, and diseases such as FIV, Feline Leukemia, and heartworms. In the event of surgical death, we will request the option to perform a necropsy (animal autopsy). If your pet receives a microchip, it is your responsibility to update all changes through HomeAgain @ www.homeagain.com or 1-888-466-3242. If your pet is ever lost from you, please report the loss to your local Humane Society/Animal Shelter and to www.homeagain.com or 1-888-466-3242. If a lost pet is found all communications will be handled through HomeAgain. I understand and certify that: The risk of injury or death during surgery, although extremely low, is always present. My pet has not had food since midnight last night. If pet is under 4 months of age, food and water is ok until 7 am. I understand the inherent risks of failing to maintain current vaccinations and waive all claims arising out of or connected with the performance of this operation due to such failure. If I do not pick up my animal at the designated time, I will be charged a $20.00 late charge and $20.00 per day boarding cost. If I do not pick my pet up the next day, Animal Allies will release abandoned pets to the Spartanburg Humane Society. It takes up to 2 weeks for vaccinations to protect my pet. I have informed Animal Allies of any injuries or illnesses my pet has. The veterinarian may note minor health issues that need to be treated & prescribe medication for treatment at an additional cost. I am under no obligation to purchase this medication. I understand that if my animal is pregnant, the pregnancy will be terminated at surgery. I understand that if my animal has an open umbilical hernia or is diagnosed as a cryptorchid. It will be repaired at time of surgery at an additional charge of $20.00. Animals 75.1-100lbs. will have an additional fee of $15 added, animals 101 - 125lbs. will have a $30 fee added, and animals 126 - 149lbs. will have a $45 fee added. I understand Animal Allies has the right to refuse surgery to any animal they feel surgery would deem a health risk. I understand Animal Allies will scan all surgical candidates prior to surgery for microchips. If microchips are detected Animal Allies will notify the microchip registry company prior to surgery for authorization of any/all services rendered. YOUR ANIMAL WILL RECEIVE A SMALL TATTOO ON HIS/HER UNDERSIDE TO SHOW THAT HE/SHE HAS BEEN STERILIZED. I hereby release Animal Allies, the veterinarians, assistants, and all of its/their officers, directors, employees, volunteers, drivers and members of staff from any and all claims arising out of or connected with the performance of this operation or procedure, and administration of vaccinations. I agree that I have not or will not claim any right of compensation from them, or any of them, or file action by reason of such sterilization or attempted sterilization of such animal or any consequences related thereto. Owner/agent hereby agrees to indemnify and hold Animal Allies harmless for any damages caused during the transportation of the animal, or for any damages caused by any unforeseeable events including fire, vandalism, burglary, extreme weather, natural disasters or acts of God.
We spay female dogs and cats by removing all of their reproductive organs (uterus, and ovaries) . We neuter male dogs and cats by removing their testicles.
Yes, all operations are performed while your pet is under anesthesia.
We will spay and neuter puppies and kittens when they are 2 months old and at least 2 lbs. Female cats can go into heat as early as 4 months of age. Female Dogs can go into heat as early as 6 months of age.
We believe in providing your pet high-quality veterinary care. Although we offer reduced rates, we do not offer low-quality services. All spay and neuter surgeries are performed by qualified, licensed veterinarians. All animals are attended to before and after surgery by trained veterinary technicians and assistants.
Your pet will receive high-quality pain medication at time of surgery to help manage their pain. Our veterinarians do recommend that you purchase three days of additional pain medication for your pets recovery time. Remember, never give your pet over-the-counter pain medications. Pain medications that humans take can be fatal to your pet!
